Applicant must be a RELA member to be eligible for application
New application: age of applicant and spouse aged from 16 to 65 years; renewal up to 70 years old (must renew before policy expiry date); If more than 1 spouse, only 1 named / declared spouse will be covered.
Age of applicant’s children from 30 days to 23 years and who are primarily dependent upon the applicant and are still studying as fulltime students
The applicant, spouse and children must be free from any physical defect and must be in normal health.

Pemohon mesti adalah Anggota RELA, sila daftar sebagai anggota RELA sebelum membuat permohonan.
Permohonan baru: umur pemohon dan pasangan di antara 16 hingga 65 tahun; Pembaharuan sehingga 70 tahun (pembaharuan perlu buat sebelum tarikh luput insurans); Jika lebih daripada 1 pasangan, hanya 1 pasangan yang dinama / diisytiharkan akan dilindungi.
Umur anak-anak Pemohon di antara 30 hari hingga 23 tahun, yang masih dibawah penjagaan pemohon merupakan pelajar penuh masa
Pemohon, pasangan dan anak-anak mesti bebas dari kecacatan fizikal dan berada di dalam keadaan sihat
